Athletic Registration for Middle School and High School sports for the 2024-2025 school year is open. All students, grades 7-12, who wish to participate in athletics are required to complete their registration in Bound.

Included in Bound registration are 4 required forms:
Heads Up: Concussion form (required by State of Iowa)
Student Conduct in Extra Curricular Activities
Guardian and Participant Acknowledgement of Risk and Release
Permission to Provide Medical Treatment Agreement
✔ School Physical Form
Students registering for athletics must have a current physical/medical examination before the student is authorized to participate in practice. This is valid for one calendar year. Please send completed physical/medical examination forms to:
